What we collect

The enquiry form on our contact page collects what you enter: your name, email address, phone number, the date of birth of the person seeking therapy, who the therapy is for, and your message. Our form provider also records the IP address, browser and referring page of each submission.

Reading the site stores one cookie on your device, which remembers whether you chose English or French.

We do not use analytics, advertising or tracking tools of any kind.

How we use it

To respond to your enquiry and, if it leads to an appointment, to arrange it. We do not use it for marketing, and we do not sell or share it. We will not disclose it to anyone else unless required by law.

Clinical records, if you become a client, are kept separately in our booking and records system.

Your rights

You may ask to see the personal information we hold about you, to correct it, or to delete it, and you may withdraw your consent to us holding it. Write to admin@jadetherapy.ca.
